Information We Need To Know From You
Our Lake Worth eyecare insurance specialists will check in advance on what your vision plan benefits are, and how often you are eligible for services and materials. Then, we will help to maximize the use of these benefits to give the most value in fulfilling your eyecare needs.
To best help you, we’ll need the following information about the person covered (the primary insured or employee) by your vision care plan.
- Name of patient (you)
- Name of primary insured
- Primary insured’s insurance ID number
- Birthdate of primary insured
- Social security number of primary insured (sometimes needed)
This information is requested by your vision care plan to allow us to check your eligibility and benefits allowances. Please have this information ready when you call to schedule your appointment. We treat your personal information with great care and keep your data private. We share it only with your vision plan providers and, if necessary, in accordance with Federal HIPAA requirements. What’s the Difference Between Health and Vision Insurance?
Your Vision Insurance is separate from your Medical Health Insurance. It’s often from a different insurance provider with which your medical health plan has a contract, and covers the annual eye exam necessary to determine if you need an eyeglass/contact lens prescription or a change to your current eyeglass/contact lens prescription. Most Vision Insurances provide a substantial allowance for a complete pair of eyeglasses OR a fixed dollar amount for contact lenses. Only very rarely are both eyeglasses and contact lenses paid for by your insurance in the same enrollment year. The contact lens coverage typically includes the professional evaluation and a limited supply of contacts.
Your Medical Health Insurance will be used to treat and manage any eye health conditions you may have. This would include eye infections, eye injuries, corneal foreign body removal, symptoms of flashes and/or floaters, dry eye problems, and eye allergies. This plan will also cover progress evaluations to monitor cataracts, glaucoma, macular degeneration, diabetic eye complications and other recurring issues.
